Pursuant to Section 5.02(D) of the Master Settlement Agreement, Takeda and the Plaintiffs' Settlement Review Committee have designated that October 7, 2015 is the Effective Date of this Program. The Claim Package Deadline expired on January 8, 2016. Therefore, the Claims Administrator is no longer accepting Claim Packages.
On January 15, 2016, the Claims Administrator made the Extraordinary Injury Claim Form for Qualifying Program Claimants available through the secure ACTOS Portal. Qualifying Program Claimants can now submit an Extraordinary Injury Claim Form regardless of how the claimant answered the EI questions in the Points Award Claim Form. The deadline to submit an Extraordinary Claim Form is Monday, July 18, 2016, which means all EI Claim Packages must be submitted through the ACTOS Portal no later than 11:59 p.m. CT on July 18, 2016.

The ACTOS Portal is a secure website that allows for the exchange of information between the Claims Administrator of the ACTOS Resolution Program and a claimant (or the authorized representative for a deceased or legally incompetent Product User) or a claimantÂ’s law firm.
Only persons authorized by the Claims Administrator may use the ACTOS Portal. If you are a lawyer requesting access to the ACTOS Portal and you are a member of a law firm that has not been granted access, click the Request Access button to obtain a Login ID and Password. If a member of your law firm has been granted access to the ACTOS Portal, that person, as the Primary Counsel for your law firm, can add new users on the Firm Details tab of the ACTOS Portal. Only law firms can use the ACTOS Portal. Claims preparation firms, accountants and other entities may not use the Portal.

claimsadministrator@actosofficialsettlement.com for information regarding the ACTOS Resolution Program.
Claimants who are represented by a lawyer cannot use the ACTOS Portal themselves and cannot receive a Login ID. Such persons should contact their lawyer for information on the status of their claims in the ACTOS Resolution Program.
